Who qualifies
A quick look at who this program is designed to help.
Must meet at least one of these criteria: aged 65 or older, have a permanent disability, or be medically fragile (conditions that are significantly complicated by poor oral health). Must be unable to afford dental care. Must not have dental insurance or adequate coverage for needed treatment.

How it works
Apply online at the Dental Lifeline Network website or call to request an application. Complete the application and submit it with documentation. If accepted, you will be matched with a volunteer dentist in your area. Wait times can be significant: apply as early as possible.
