Have these ready before you start the application.
- Completed financial assistance application, proof of household income (pay stubs, tax return, W-2, Social Security benefit letter, or unemployment documentation), proof of identity (driver's license, state ID, or passport), proof of residency (utility bill or lease), and any medical bills or explanation of benefits (EOB) statements.
How it works

Complete the application with household income, family size, and insurance information. 3. Attach required documentation (income verification, identification). 4. Submit the application to the hospital's financial assistance office. 5. The hospital will review and notify you of the decision, typically within 30 days. 6. If approved, the discount or write-off will be applied to your account.
