Must be a US resident. Must have health insurance (Medicare, commercial, or private). Household income must be at or below 500% of the Federal Poverty Level (varies by disease fund). Must have a diagnosis covered by one of the open disease funds. Must have out-of-pocket costs for treatment.

- Insurance card or proof of coverage, proof of income (tax return or pay stubs), diagnosis confirmation from your healthcare provider, and prescription information.
How it works
Apply online at the HealthWell Foundation website. Check if the fund for your diagnosis is currently open. Complete the application with your insurance and income information. If approved, grants are applied directly to your pharmacy or healthcare provider.
