Who qualifies
A quick look at who this program is designed to help.
Must be a US resident. Must be uninsured or functionally uninsured (no prescription drug coverage that covers the requested Lilly medication). Individual income must be at or below 400% of the Federal Poverty Level. Must not be enrolled in or eligible for any government-funded insurance program such as Medicare Part D, Medicaid, or VA benefits.

What you'll need
Have these ready before you start the application.
- Completed application signed by patient and prescribing physician, proof of income (tax return, pay stubs, or Social Security letter), and a valid prescription for the Lilly medication.
How it works
Your healthcare provider must complete and submit the Lilly Cares application. Applications can be submitted online, by fax, or by mail. Once approved, medications are shipped directly to your doctor's office. Re-enrollment is required every 12 months.
