Prescription Advantage helps eligible residents of MA pay for prescription medications. Income limits apply at 500% of the federal poverty level. Applicants must be 65 years or older.

- Completed application form, proof of income (Social Security benefit letter, tax return, or pension statement), proof of state residency, proof of age (driver's license or birth certificate), and Medicare card or proof of Medicare Part D enrollment.
