Affordable orthodontic treatment ($695 total) for low-income youth.
Connects children and teens from low-income families with orthodontists who provide braces at a dramatically reduced fee of $695. Income must be at or below 200% FPL.

Children and teens ages 10-18. Household income at or below 200% FPL. Must have orthodontic need as determined by a participating orthodontist. Must not currently be in orthodontic treatment.

Have these ready before you start the application.
- Completed online application. Proof of household income (tax return or pay stubs). Photos of child's teeth (instructions provided in application). $25 application fee.
How it works
1. Complete an online application at smileschangelives.org. 2. Pay a $25 application fee. 3. If approved, you will be matched with a participating orthodontist. 4. Visit the orthodontist for an evaluation. 5. If accepted, treatment begins with a total cost of $695.
