KHC provides medications, transportation, and financial assistance for chronic dialysis or kidney-transplant recipients who are Texas residents. Covers ESA, phosphate binders, anti-rejection drugs, and diabetes medications common in the ESRD population. Enrollment through participating dialysis centers and transplant clinics.
